Abstract

This article describes a natural method for requesting information from the user of an interactive application. It is the electronic counterpart of a real questionnaire form. The user would be presented with a graphical categorized form of a questionnaire. It would have labeled tabs, each representing the sections (ie. home address, work experience, education, health records, etc.) The user can turn to the section he/she desires and fill out or change any information necessary. After a section has been completed, the tab for that section may be changed in color to reflect that event.

Graphical Categorized Form of Questionnaire

This article describes a natural method for requesting information from the user of an interactive application. It is the electronic counterpart of a real questionnaire form. The user would be presented with a graphical categorized form of a questionnaire. It would have labeled tabs, each representing the sections (ie. home address, work experience, education, health records, etc.) The user can turn to the section he/she desires and fill out or change any information necessary. After a section has been completed, the tab for that section may be changed in color to reflect that event.

In a windowing environment, the standard method of requesting information is through the use of dialog boxes. In many cases the prompts needed to request the information take numerous dialog boxes. The user has to go sequentially through all these dialog boxes in order to answer questions. This is both a slow and unattractive method for obtaining large amounts of data. It is difficult for the user to go back and change a response after going past a dialog box.

The Graphical Categorized form of questionnaire is implemented using a graphical form of a list control. This list control would be in the form of a tabbed questionnaire. Each tab would have the title of that section of the questionnaire. Whenever the user selects an item the application would present the user with the related page of the questionnaire.
