Abstract

We propose a system and method to provide a relatively reliable flow of infusate through all holes of a multiholed catheter, at relatively low overall rates of administration. In such a system and method, a relatively uniform or even spread of delivery of an infusate out of each hole of a multihole catheter would result from a relatively continuous rate of delivery of an infusate over time but in relatively fast, high pressure bolus-like pulses from an infusion pump. The rates and amplitudes of the pulses could be adjustable, for optimization of even spread delivery from each hole of a multihole catheter in various infusion protocols; and time intervals between the pulses could be shortened with corresponding increases in infusion rates. The system and method would be particularly advantageous in, for example, an epidural infusion which utilizes a multihole catheter to optimize the epidural effect for a patient.

Abstract

Disclosure

    In the medical arts, infusion pumps have become useful devices for managing the delivery and dispensation of therapeutic medications to patients. Infusion pumps can provide some significant advantages over manual infusion techniques, by accurately delivering and dispensing infusates over an extended period of time. Infusion pumps can be particularly useful for treating diseases and disorders that require regular pharmacological intervention, including cancer, diabetes, and vascular, neurological, and metabolic disorders. Infusion pumps can also enhance the ability of healthcare providers to deliver anesthesia and manage pain, through various delivery methods, which may include intravenously, intra-arterially, subcutaneously, intraperitoneally, in close proximity to nerves, and into an intraoperative site, epidural space or subarachnoid space.

    In some infusion treatments and therapies, it may be desirable to provide a continuous rate of delivery of a medication from the device to the patient. In such continuous rate infusion protocols, it is usually advantageous to match an instantaneous rate of delivery to a rate of delivery over a relatively longer period. An infusion profile in which the instantaneous rate of delivery approximately matches the rate of delivery over the longer period is often referred to as one having "flow constancy". However, for other profiles such as epidural delivery through a

multiholed catheter, flow constancy may not be important and may actually be undesirable. Multihole catheters are designed to deliver fluid over a greater area of the epidural space than single-hole catheters; but low-rate continuous infusions, such as epidural anesthesia infusions, tend to not uniformly produce flow outwardly from all holes of a multihole catheter.
