A method and system is disclosed to provide a comprehensive medication management system to improve general health of the population.

Numerous factors contributing to the erstwhile sub-standard medication management systems may be, but are not limited to patients' sub-optimal health literacy, lack of involvement in treatment decision-making process, prescription of complex drugs regimens, communication barriers, ineffective communication of information about adverse effects, provision of care by multiple physicians and limited access to care and health information technology. There exists a need for a system for effective integrated medication management with information from relevant sources, timely interventions and enhanced medical adherence.

Disclosed is a system and method to provide a comprehensive medication management system to improve general health of the population. The method and system provides a HIPAA (Health Insurance Portability and Accountability Act of 1996) compliant cloud platform to facilitate a personal medication management system, where

in the system receives medication and other health related information from Pharmacy Benefit Management service providers, health systems and links this information with other information sources and various delivery channels.

A comprehensive medication adherence system requires integration of a number

independent systems that pool their resources and capabilities together. These are reusable, interchangeable, replaceable, extensible, integrable components that make up the essential building blocks of a framework for medication adherence applications. Our disclosure defines the components and their integration points as the basic structure of an underlying system of systems that allows developers to develop and deploy a myriad of application for medication adherence and management.

The system of systems described here brings together patient data sources, health related data sources, intervention channels, consent management system, analysis and insight generation capabilities to enable the development of a myriad of medication adherence application.

As illustrated in the figure above, the system enables access to patient data from different providers through plug-ins. Medication, health, behavioral and social data providers register to the system through "Patient Data Provider Registration" and agree to share data. "Consent management" subsystem enables patients, data providers and channel owners to affirm their participation and establish consent directives to determine who will access to PHI, for what purpose and under what circumstances and how. Patients can opt-out some of the services they will receive from the system through "Patient Preference" management system.
