A bar code scanner may use a rotating glass disk which carries an annular array of beam-deflecting holographic segments or holofacets. If a complex scan pattern is to be generated using a set of beam-folding mirrors, the beam may be deflected to different elevations relative to the surface of the disk but also at different skew angles (forward or backward in the direction of disk rotation) relative to the point at which the incoming beam is incident on the disk.
