When handling or transporting tape cartridges, the outer layers of the tape tend to get loose due to vibration. When a new tape operation is attempted, the sudden tension created by the column vacuum and applied to the loose portions causes a double tape crease (a Z shape) to appear. Therefore, the tape is mechanically destroyed, causing permanent read errors.
