In medicine, adherence (or compliance) refers to the degree to which a patient correctly follows prescriptive medical advice. Generally, this refers to drugs, but it can also apply to medical device use, exercise, or diet. Non-adherence is widely recognized as a major component of healthcare costs today. Existing solutions to non-adherence include educating patients about side effects, providing mobile reminders to take medicine, and more.

Adherence is a difficult problem because patients may make a choice not to follow the prescription for a variety of reasons, including the prescription's excessive demands on their time, their physical disabilities, fear of side effects, daily stress, mood swings, or pain. In some cases, if a patient complains, doctors are able to moderate (slightly modify) the medical advice to account for the individual patients' circumstances. However, this is manual, error prone, slow, and expensive. In most cases, the doctor is unaware of patient circumstances and thus has no way to change the medical advice. In other cases, patients may not report or remember their obstacles to adherence, making it impossible for doctors to respond.

Increasingly, however, patients are able to use medical devices that record or measure progress toward health goals, such as achieving a given weight, blood glucose level, or blood pressure. Patients can also increasingly use computer applications (apps) and mobile apps that provide a way to report health-related data such as what the person ate, the duration and intensity of exercise, and the medicine taken.

An automated method of moderating medical advice using data recorded from patients is desirable. Such a system can allow a patient fewer doctor visits, greater satisfaction with health care, and increase the likelihood of adherence to medical advice.
